ACENAC Tablet contains Aceclofenac which is a preferential COX 2 inhibitor. Exhibits multiple mechanisms of action. It blocks the prostaglandin (PGE2) secretion and inhibits the release of Cytokines, as well as it stimulates the synthesis of cartilage matrix and blocks its degeneration.

Besides this it increases hexosamine level, a gastro protective agent, hence GI side effects are less associated with aceclofenac.

Pain is a complex phenomena involving both peripheral and central mechanism. The combination of aceclofenac with paracetamol (Acenac-P) gives complete relief from pain and associated inflammation by counteracting both central and peripheral mechanisms.

Aceclofenac can counteract peripheral inflammation by blocking COX-2 enzyme and paracetamol blocks the central effects of prostaglandin by blocking COX-3 enzyme in brain.

This combination offers faster and longer duration of action with least incidence of GI side-effects. It works hard on pain, hardly on stomach.

Cyclooxygenase-2 inhibitors effectively relieve osteoarthritis symptoms when taken in the morning and better results are obtained in rheumatoid arthritis when it is taken in the evening.

For achieving longer duration of action and better compliance, drug levels are required to be maintained above effective therapeutic concentration for 24 hours in a day. Sustained release system achieves slow release of drug, which maintains the drug levels in blood or target tissue over an extended period of time.

So it provides better efficacy in once daily dose for providing symptomatic relief in chronic painful conditions like Osteoarthritis, Rheumatoid Arthritis & Spondylitis.

Mixed pain syndrome is associated with conditions like low back pain, sciatica, radiculopathy (cervical & lumbar) & chronic post operative pain. Aceclofenac takes care of nociceptive (inflammatory) pain by inhibiting prostaglandin synthesis. Pregabalin binds to α2-δ subunit of voltage gated calcium channels & reduces the release of neurotransmitters like glutamate and substance P. This reduction in neurotransmitter release accounts for pregabalin’s analgesic action.

Both component of mixed pain syndrome nociceptive (inflammatory) & neuropathic pain are taken care by aceclofenac & pregabalin respectively.

Each enteric coated tablet contains:
Trypsin - 48 mg
Bromelain - 90 mg
Rutoside Trihydrate - 100 mg

Adults: 1 to 2 tablets every 8 hours preferably one hour before meal or as directed by the physician.
Children: 1 Tablet/10 kg per day preferably 1 hour before meal or as directed by the physician.

Tissue injury releases the chemical mediators like prostaglandin & bradykinin which causes inflammation, edema and pain. Combination of enzyme therapy is required to take care of inflammation, edema and pain.

In Eezflam, bromelain is a cysteine protease & trypsin is a serine protease. The combination of serine & cystein protease is logical, as bromelain acts on alanine, tyrosine & glycine and trypsin works on lysine & arginine.

Bromelain breaks down plasma proteins that have exuded into the extra vascular tissue, thus exerting anti-edema action. Bromelain reduces level of prostaglandin, bradykinin & proinflammatory cytokines which manifest its analgesic and anti-inflammatory effect.

Trypsin exerts a fibrinolytic action, thereby opening the blocked blood vessels & lymph vessels which lead to reduction of edema and inflammation. Trypsin induces phagocytosis & removes cell debris which causes faster healing of wound.

Rutin stabilizes vascular endothelium & reduces leakage of fluid from blood vessel into the surrounding tissue, thus helping in quick resorption of edema.

Bromelain and trypsin besides being an anti-edematous anti-inflammatory agent, is also an immunomodulatory agent that breaks down excess cytokines in the vicinity of the inflammatory site, thereby reducing the intensity of the inflammation.

Together these three drugs reduce edema, inflammation and hence the pain induced by the pressure of exudates on nerve endings.

Adults: 1 tablets twice daily preferably one hour before meal or as directed by the physician.

Sinusitis, Otitis media.

Very rarely there have been allergic reactions that disappear after discontinuation of combination therapy.
