Description
Metoprolol is a beta 1 -selective (cardio selective) adrenergic receptor blocking agent, for oral administration, available as extended release tablet. BETABEST-XL has been formulated to provide a controlled and predictable release of metoprolol for once daily administration.

Dosage
Hypertension
The usual initial dosage is 50 to 100 mg daily in a single dose. The dosage may increase at weekly intervals until optimum blood pressure reduction is achieved.
Angina pectoris: The dosage may be gradually increased at weekly intervals until optimum clinical response has been obtained.
Heart failure
The recommended starting dose of BETABEST-XL is 25 mg once daily in patients with NYHA class II heart failure. The dose should then be doubled every two weeks to the highest dosage level tolerated by the patient.
Contraindications
Severe bradycardia, Heart block greater than first degree, Cardiogenic shock Refractory heart failure, Sick sinus syndrome (unless a permanent pacemaker is in place), Hypersensitivity to any component of this product

Description
It is the fixed dose combination of amlodipine (calcium channel blocker) and metoprolol (cardioselective beta blocker); both the drugs have two different mechanisms and reduce blood pressure by acting on peripheral vascular resistance, stroke volume and heart rate. Advantages of this combination therapy are :It effectively achieves target Blood Pressure, Lower incidence of individual drug’s side-effects, produces synergistic effects, increased patient compliance.

Encardil 2.5/5/10
Description
Encardil contains Enalapril. Enalapril is an antihypertensive agent. It belongs to the second generation of Angiotensin Converting Enzyme (ACE) inhibitors. Enalapril is a prodrug. It gets hydrolysed in the liver to its active form Enalaprilat, which inhibits the Angiotensin Converting Enzyme.

Dose
Hypertension: Therapy can be initiated with 2.5 - 5 mg O.D. Maintenance dose is 5 to 20 mg O.D. In elderly patients with impaired renal function, it can be started with 2.5 mg O.D.

Congestive heart failure: Start with 2.5 mg O.D. and can be increased to 10 to 20 mg daily. Post MI: Dose has to be titrated as per the requirement.
Side effects
Encardil is well tolerated. Some of the mild side effects include cough, hyperkalemia and skin rash.

Nevol 2.5/5
Description
Nebivolol is a selective B1-receptor antagonist. Blocking the beta-receptors in the heart causes the heart to beat more slowly and with less force. This means that the pressure at which blood is pumped out of the heart to the rest of the body is reduced.

It has mild vasodilating properties, possible due to an interaction with the L-arginine/nitric oxide pathway, which widens the blood vessels.

Nebivolol with its distinct mechanism of action is very good antihypertensive and useful for the treatment of mild to moderate hypertension.
Indications
Treatment of mild to moderate essential hypertension
Composition
Each tablet contains:
Nebivolol Hydrochloride   2.5/5mg
Dosage
5 mg tablet once daily, preferable at the same time of the day, with or without meals.
Patients with renal insufficiency the recommended starting dose is 2.5 mg daily.
The daily dose may be increased to 5 mg if needed.
Elderly patients over 65 years, the recommended starting dose is 2.5 mg daily. The dose may be increased to 5 mg daily if needed.
Side Effects
Headache, bradycardia, diarrhea, constipation, nausea, vomiting or abdominal pain.

Nevol H
Description
Hypertension is a multifactorial disease. One drug addresses only one physiological pathway of many that leads to hypertension.

The other mechanisms of blood pressure regulation will be stimulated and play major role in raising the B.P. Hence two or more agents from different pharmacological classes are often needed to achieve adequate blood pressure control.

Nebivolol has dual mode of action: It is a competitive & selective β1-receptor antagonist & it has mild vasodilating properties.

Nebivolol reduces heart rate & blood pressure at rest & during exercise. Acute effects of thiazides are as a result of a reduction in blood volume and cardiac output, secondary to a natriuretic effect. Better blood pressure control can be achieved by using this combination.

Management of hypertension not responding to monotherapy.
Hypertension with left ventricular failure.

Dosage

Primodil 2.5/5/10 MG
Description
Primodil (Amlodipine) is the second generation, long-acting calcium channel blocker. It is more selective for vascular smooth muscle than for myocardial tissue. It acts by primarily inhibiting the entry of calcium ions into the vascular smooth muscle.

As we know, contractile process of smooth muscle is dependent upon the entry of extra cellular calcium ions into these cells through Ca++ channels. It is more selective for arteries.

Thus, Amlodipine is also termed as arterial vasodilator. Because of this mode of action Primodil is effective in the management of mild to moderate hypertension as well as in both stable angina and vasospastic angina.

Dosage
For hypertension and angina, Primodil is given in the dose of 5 mg once daily.
After two weeks, dosage may be increased to a maximum of 10 mg once daily.

Side Effects
Amlodipine is well tolerated. Rarely can it cause ankle edema, headache, dizziness and somnolence.

Primodil AT
Description
In patients with mild to moderate hypertension unresponsive to monotherapy with Amlodipine, additional therapy with Atenolol further improves B.P. control. Amlodipine in Primodil-AT reduces B.P. by reducing peripheral resistance. Atenolol in Primodil-AT reduces B.P. by reducing heart rate and force of myocardial contraction; this reduces cardiac output, which in turn reduces blood pressure. The combination of Amlodipine and Atenolol is also very effective in angina because in Primodil-AT, Amlodipine increases oxygen supply to myocardium, while Atenolol decreases oxygen demand.

Primodil – L
Description
Primodil-L is a fixed dose combination of losartan (an angiotensin-II blocker) and amlodipine (a long acting calcium channel blocker) for the management of mild to moderate hypertension where the B.P is not adequately controlled by individual agents alone.

Agents acting on renin angiotensin system and calcium channel blockers have an established role in the management of hypertension. Drugs of these two classes reduce B.P by complimentary mechanisms and offer distinct therapeutic advantages.

These two potent antihypertensives coupled with less likelihood of side effects would be able to achieve the desired goal of maintaining greater reduction in B.P.

Telmed 20/40/80 MG
Description
Telmisartan, a unique angiotensin II receptor antagonist used for the treatment of hypertension, also functions as a partial agonist of PPAR gamma receptor; and reduce glucose, insulin, and triglyceride levels. None of the other available angiotensin II receptor antagonists activate PPAR when tested at concentrations typically achieved in plasma with conventional oral dosing.

In contrast to ordinary antihypertensive and antidiabetic agents, molecules that can simultaneously block the angiotensin II receptor and activate PPAR have the potential to treat both hypertension and biochemical features of the metabolic syndrome and could provide unique opportunities for the prevention and treatment of diabetes and cardiovascular disease in high-risk populations.

TELMED –AM
Description
The goal set by JNC VII in diabetic hypertensive is 130/80 mm Hg that is very difficult to achieve with monotherapy.

However, older antihypertensive agents such as thiazide diuretics and beta-blockers have potential adverse effects on glucose and lipid metabolism and may even exacerbate the metabolic syndrome and increase risk of type II diabetes.

Thus, there is a need for an antihypertensive drug combination that can achieve the JNC VII recommended goal without compromising the lipid profile and glycaemic control.

Telmisartan plus Amlodipine are two potent antihypertensives with less side effects that would be able to achieve the desired goal of maintaining greater reduction in B.P.

TELMED –H
Description
Diuretics are effective antihypertensive drugs. In long-term trials, diuretics have been shown to reduce the incidence of stroke, congestive heart failure, coronary artery disease and total mortality from cardiovascular disease.

Telmisartan exerts antihypertensive effect by selectively blocking the angiotensin II type 1 (AT1) receptors. Hypokalemia caused by diuretics, counteract the positive cardiovascular benefits of blood pressure reduction. Telmisartan protects against potassium depletion, a common side effect of thiazide monotherapy.

Thus low dose hydrochlorothiazide in combination with telmisartan does not counteract the positive cardiovascular benefits of blood pressure reduction and helps in achieving the desired goal of BP.

Description
Hypertension being a multifactorial disease, management of hypertension often requires multiple drugs. 23%–52% patients require three or more antihypertensive agents for BP control and to achieve JNC VII target.

Combining drugs with complimentary modes of action is practically correct, as it is more likely to achieve better BP control. Combining drugs reduces the adverse events of the high dose monotherapies.

A triple drug combination therapy would be a desirable treatment option for hypertension uncontrolled on dual drug.

Combining different drugs in a single pill leads to better compliance and hence better BP control.

VAZORTAN-H
Description
Monotherapy can achieve target B.P. reduction in only 30-50% patients. Even increasing doses in monotherapy may or may not achieve target B.P. reduction and may be associated with adverse reactions.

Hence this combination (Losartan & Hydrochlorothiazide) with different mechanism of actions has additive antihypertensive effect. Hydrochlorothiazide in low doses decreases incidences of cardiovascular complications.

Also losartan reverses the hypokalemia and hyperuricemia associated with Hydrochlorothiazide.

Moderate to severe hypertension.
